How to schedule:

• Schedule your appointment through the patient portal https://seattleu.medicatconnect.com/
• If having difficulty getting onto the patient portal try a different browser and emptying the cache. If continued issues, please call the SHC at 206-296-6300.
• We are asking students to schedule appointments to make physical distancing easier and minimize wait time, but we want you to have the opportunity to be vaccinated prior to traveling home for the summer. So just come during clinic hours and we will get you worked into the schedule.

What to bring to the clinic:

• SU ID
• Safe start email showing you have approval to be on campus
• Required forms (extras will be available but you will save time if you bring the completed form)
  • Insurance Attestation Form
  • Walmart consent form
