COVID-19 vaccination is required for Seattle University students, faculty, and staff. Once you have received a single dose of a one-dose vaccine, or both doses of a two-dose vaccine, you must register your vaccination details using this form. Submission of this form with adequate documentation fulfills the vaccination reporting requirement.
